About Us
CHC Direct is led by a team of directors with a wealth of experience in commissioning, implementing and maintaining compliant CHC services that reflect CHC Direct’s vison and values.
Joanna Hirst
Joanna is an experienced and resilient healthcare leader specialising in community and primary care. Joanna combines in depth knowledge of continuing and complex healthcare with a strong background in transformational, strategic and operational management at board and senior management level for both the NHS and independent sectors. As a registered clinician, District Nurse, and clinical practice development Joanna also retains broad insight and understanding of the day to day realities of CHC service delivery. Joanna has undertaken CHC decision making reviews and audits on behalf of CCGs and NHS England work streams as well as working in a range of interim roles in the NHS and independent healthcare organisations.
Catherine Crocker
Catherine as Director of Operations for CHC Direct is a highly competent and successful business analyst and Administrator, who has considerable experience of continuing healthcare as well as commercial entities in retail merchandising and trading. She has a proven track record as problem solver and is solution focussed. She is creative, driven and self-motivated with a great enthusiasm for managing through change and ensuring project success. Catherine is key to the project delivery of all CHC Direct services and has designed, managed and delivered large scale projects on behalf of CCGs including Local Resolution Meeting preparation and management Projects; Fast Track Service Review Projects; and bespoke Commissioning Review Projects.
Norma Lee
Norma brings over 38 years’ experience of working in clinical practice and in operational and strategic management within both the NHS, independent and third sector. She can also point to a proven track record in service transformation and redesign and in new business development and mobilisation. A health visitor by background, Norma’s recent achievements include implementing innovative care home and home care services, and leading the successful delivery of community service cost improvement plans, for NHS Trusts in south-west England. She also acts as a Trustee for a number of Dorset-based charities that support both home care and independent living for people with mental healthcare needs and learning disabilities.

Featured Case Studies
The Clinical Commissioning Group CCG provides CHC Services to six CCGs
We initially undertook a Time and Motion Study looking at roles and activities within the department to assist the management team in restructuring the team and transforming the service.
Moving with the times - Covid-19
Covid 19 has impacted us all in so many different ways. CHC Direct has, in common with our customers, amended our working practices to enable us to continue to deliver high quality services to customers, individuals being assessed and their families and representatives.
We adapted our service delivery models by undertaking assessments using video conferencing ensuring we can interact with individuals being assessed, their care providers and their families and representatives.
With the changing government guidance we are able to complete some assessments face to face again, but we continue to deliver technology enabled assessment and other services where it is appropriate to do so.

The National Framework for NHS Continuing Healthcare and NHS funded Nursing Care (revised 2018) provides a pathway and guidance for the delivery of continuing healthcare services.
The revised version of the Framework is intended to:
- provide greater clarity to individuals and staff, through a new structure and style
- reflect legislative changes since the 2012 National Framework was published, primarily to reflect the implementation of the Care Act 2014,
- clarify a number of policy areas
