- CHC Direct Consultancy – independent and objective advice about local CHC processes (including assessment, verification and commissioning processes)
- Hands on support for service improvement/transformation – including process mapping and service and process design
- Hands-on support for change – assisted by our highly experienced Associate interim managers and senior clinicians
- Hands on support for CHC Assessments – our fully-trained and experienced Associate assessors can work as part of your team, or as a fully outsourced solution (checklist completion; Multidisciplinary Team (MDT) CHC assessment using the Decision Support Tool (DST); fast track reviews; CHC and FNC Reviews; PHB assessment, set up and review)
- Delayed Transfers of Care (DTOC) Reduction – to manage/reduce NHS-attributable DTOCs arising from a lack of timely CHC Assessment, with experienced CHC Assessors on hand to deliver assessments:
- On a per assessment basis
- During episodes of peak demand
- During periods of staff absence
- As a regular service activity – so eligibility decisions are consistently made within the 28-day window
- Discharge to Assess – design, commissioning and/or operational management of the NHS England Quality Premium Standards
